Rilmazafone
Rilmazafone is a water-soluble benzodiazepine prodrug developed and marketed in Japan as a hypnotic. It is inactive until metabolised in the gut to an active benzodiazepine. It has genuine Japanese clinical use as a sleep aid, but is treated as a research chemical elsewhere and shares the dependence profile of the class.
01 Overview
Rilmazafone is a prodrug that is itself inactive; intestinal enzymes cleave it to an active benzodiazepine metabolite. Its water solubility and prodrug design were intended to improve tolerability as a hypnotic, and it has been used clinically in Japan for insomnia. Outside Japan it circulates as a grey-market research chemical.
Reported and clinical effects are those of a benzodiazepine hypnotic: sedation, sleep induction, anxiolysis and muscle relaxation. Because activation depends on gut metabolism, onset is somewhat delayed relative to standard benzodiazepines. Dependence, tolerance and withdrawal risks remain, as does additive depression with other sedatives.

Prodrug hydrolysed by intestinal aminopeptidase to an active metabolite that positively modulates GABA-A receptors at the benzodiazepine site.
