Pseudoephedrine is a sympathomimetic amine and stereoisomer of ephedrine, used as an oral decongestant. It is milder than ephedrine but shares its cardiovascular and stimulant effects, and is used off-label and in supplements as a modest thermogenic and appetite suppressant. Because it is a methamphetamine precursor, its retail sale is legally restricted in many countries.

01 Overview

Pseudoephedrine is the (+)-threo diastereomer of ephedrine. It acts mainly indirectly, releasing noradrenaline, with weaker direct adrenergic activity than ephedrine. Its principal medical use is as an oral nasal decongestant, constricting swollen nasal blood vessels via alpha-adrenergic stimulation.

Because it raises sympathetic tone, it produces a mild thermogenic and appetite-suppressant effect and is sometimes used off-label or in combination supplements for fat loss, though it is weaker than ephedrine for this purpose. Side effects are the sympathomimetic set: raised blood pressure and heart rate, insomnia, jitteriness and, rarely, more serious cardiovascular events. As a methamphetamine precursor, its sale is tracked and restricted under precursor-control laws.

02 Mechanism

Indirect-acting sympathomimetic that promotes noradrenaline release with some direct alpha- and beta-adrenergic activity. Alpha-mediated vasoconstriction relieves nasal congestion; increased sympathetic tone produces mild thermogenesis and appetite suppression.
