Enclomiphene is the trans-isomer of clomifene, isolated from the mixed drug to keep the potent oestrogen-antagonist activity while dropping the long-lived zuclomiphene isomer responsible for many of clomifene's side effects. It raises LH, FSH and testosterone in men with secondary hypogonadism and clears quickly, making it a cleaner SERM for raising testosterone while preserving fertility.

01 Overview

Enclomiphene provides the pituitary oestrogen-antagonism of clomifene without the accumulating zuclomiphene fraction. In clinical trials in men with secondary hypogonadism it raised testosterone into the normal range while maintaining or improving sperm concentration — a contrast to testosterone replacement, which suppresses spermatogenesis.

Because it lacks the persistent oestrogen-agonist isomer, it tends to produce fewer visual and mood side effects than clomifene, though it was never brought fully to market under its intended brand and much supply is via compounding or research channels.

02 Mechanism

Trans-isomer SERM that antagonises oestrogen receptors at the hypothalamus/pituitary, raising LH, FSH and endogenous testosterone with rapid clearance.
