Darbepoetin Alfa
Darbepoetin alfa is a hyperglycosylated, longer-acting analogue of erythropoietin with two extra N-linked carbohydrate chains, extending its half-life so it can be dosed less frequently. Licensed for anaemia, it has been detected in high-profile doping cases (notably at the 2002 Winter Olympics). Prohibited by WADA at all times.
01 Overview
Engineered with five N-linked glycosylation sites (versus three in native EPO), darbepoetin has a roughly threefold longer terminal half-life, allowing weekly or fortnightly dosing for renal and cancer anaemia. Its mechanism is identical to EPO: stimulation of the EPO receptor and expansion of red cell mass.\n\nIts longer action and distinct glycosylation initially made it attractive to dopers, but it is readily distinguished from endogenous EPO on isoelectric focusing and was central to positive tests among cross-country skiers in 2002. The cardiovascular risks mirror EPO: thrombosis, hypertension and, in oncology trials, signals of increased mortality and tumour progression at high haemoglobin targets.
02 Mechanism
Binds and activates the erythropoietin receptor exactly as native EPO, but its additional sialic-acid-rich carbohydrate chains slow clearance, giving a longer duration of erythropoietic stimulation per dose.
