Cobalt Chloride
Cobalt chloride is an inorganic salt that stabilises HIF and stimulates erythropoietin, historically used as a treatment for anaemia before EPO existed. It is a cheap, orally available hypoxia mimetic exploited in doping, but is toxic — causing cardiomyopathy, thyroid dysfunction and neurotoxicity. WADA prohibits cobalt as a HIF activating agent.
01 Overview
Cobalt ions inhibit HIF prolyl hydroxylases and mimic hypoxia, raising erythropoietin and haemoglobin. Cobalt salts were used medicinally for anaemia in the mid-20th century until toxicity, notably an epidemic of fatal cardiomyopathy among heavy beer drinkers whose beer was cobalt-stabilised, ended the practice.\n\nBecause it is inexpensive and orally active, cobalt is misused as a doping agent, and WADA explicitly lists it. The problem is a narrow, unpredictable margin between an erythropoietic dose and a toxic one: cobalt accumulates and causes cardiomyopathy, hypothyroidism, goitre, hearing and vision damage and possible carcinogenicity. It is not a legitimate performance aid.
02 Mechanism
Cobalt ions inhibit HIF prolyl hydroxylases and substitute for iron in oxygen-sensing enzymes, stabilising HIF-alpha and increasing erythropoietin transcription; they also impair cellular oxygen utilisation, contributing to toxicity.
