Bloodwork is the only objective feedback you get while using anabolic steroids. Everything else is a mirror and a feeling. A panel does not make a cycle safe, but it turns invisible strain into numbers you can act on before it becomes a symptom. This guide walks the markers worth running, sensible timing, and how to interpret the changes that show up on almost every cycle.
Timing matters as much as the markers. Draw a full baseline panel before you start anything, ideally in the morning and fasted. On cycle, mid-cycle labs (around week 6-8 of a longer run) catch problems while there is still time to adjust. If you use an oral, run liver and lipid markers within 4-6 weeks of starting it. Post-cycle, wait until esters have cleared before judging your recovery: testosterone enanthate has a half-life near 4.5 days, so meaningful clearance takes roughly 5-6 weeks after the last shot before an LH/FSH/testosterone panel reflects your own axis.
The hormone panel: total and free testosterone
Total testosterone tells you roughly how much you are running; free testosterone tells you how much is biologically active. A natural male reference range is broadly 264-916 ng/dL (varies by assay). On a moderate 500 mg/week testosterone enanthate cycle, total testosterone commonly lands somewhere in the 1,500-3,000 ng/dL range at peak, though this depends heavily on timing relative to your injection and the assay used. The number itself is less useful than the trend and its relationship to your other markers. If total testosterone is very high but free testosterone is disproportionately low, SHBG is likely elevated and pulling more of the hormone into a bound, inactive state.
Request free testosterone measured by equilibrium dialysis where you can. Calculated free testosterone (from total, SHBG, and albumin) is cheaper and usually adequate for tracking trends.
Estradiol (E2)
Aromatase converts a fraction of testosterone into estradiol, and more substrate means more estradiol. The sensitive (LC-MS/MS) estradiol assay is the one to order for men; the standard immunoassay is designed for the female range and reads unreliably in men. There is no single 'correct' on-cycle E2. Estradiol tracks dose, and symptoms matter more than a target number: water retention, moodiness, and nipple sensitivity suggest it is high relative to your androgens, while flat mood, no libido, joint pain, and poor erections suggest it has been crushed too low. Chasing a specific figure with an aromatase inhibitor is how most people over-suppress and feel worse. Adjust to symptoms, confirm with the sensitive assay.
LH and FSH
Luteinizing hormone and follicle-stimulating hormone are the pituitary signals that drive your own testosterone and sperm production. On any suppressive cycle they go to the floor, often <0.1 IU/L; that is expected and not informative during the cycle. Where LH and FSH earn their place is at baseline and after PCT. Recovery means these climbing back into range (roughly LH 1.7-8.6 IU/L, FSH 1.5-12.4 IU/L) and dragging endogenous testosterone with them. If they stay flat weeks after clearance, your axis has not restarted.
Lipids
See the companion heart guide for why this is the single most important cardiovascular marker on cycle. Briefly: androgens, especially orals like oxandrolone and stanozolol, suppress HDL sharply and raise LDL. HDL can drop from a healthy 50-60 mg/dL into the teens on strong oral cycles. Run a full lipid panel at baseline and mid-cycle, not just at the end.
Hematocrit and hemoglobin
Testosterone stimulates red blood cell production. Hematocrit (the percentage of blood volume made of red cells) commonly climbs several points on cycle. The usual male range is roughly 41-50%; sustained values above ~52-54% raise viscosity and are the trigger point many clinicians use to intervene. Track hematocrit and hemoglobin together. Rising values with symptoms like headaches, flushing, or visual disturbance warrant action, covered in the heart guide.
Liver enzymes: ALT and AST
ALT and AST rise on oral (17-alpha-alkylated) cycles. The catch, detailed in the liver guide, is that trained lifters leak AST and ALT from muscle too, so a modest elevation after heavy training is not automatically hepatic. GGT is more liver-specific and is worth adding when ALT/AST are up but you suspect muscle is the source. Reference ranges vary by lab but ALT and AST under ~40 U/L are typical; on orals, two to three times that is common and usually reverses on discontinuation.
Prolactin
Prolactin is mainly relevant if you run 19-nor compounds (nandrolone, trenbolone), which can raise it and cause low libido, erectile difficulty, or nipple discharge. On a plain testosterone cycle it rarely moves much. Male reference is roughly 4-15 ng/mL. If it is elevated and symptomatic, that points toward a different management path than estrogen. A markedly high reading with no drug explanation warrants a medical workup, not a guess.
PSA and prostate
Prostate-specific antigen can rise modestly with androgen use. It is most relevant for men over 40 as a baseline and monitoring marker. Steroids do not appear to cause prostate cancer, but they can accelerate an existing prostate issue, so a rising PSA deserves a physician's assessment rather than self-management.
Fasting glucose (and HbA1c)
Some compounds and the weight gain that accompanies a bulk can worsen insulin sensitivity; growth hormone and high-dose orals are the usual culprits for glucose creep. Fasting glucose above ~100 mg/dL, or HbA1c drifting up, is worth watching. HbA1c reflects roughly three months of average glucose, so it is a good slow-moving check across a cycle.
Reading the panel as a whole
No marker is meaningful alone. High testosterone with tanked HDL and a rising hematocrit is a different situation from high testosterone with stable lipids. Look at direction of travel against your own baseline, not just whether a value sits inside a population range. Keep every panel so you can see trends across cycles.
Bottom line
Baseline before, mid-cycle to catch problems early, and post-clearance to confirm recovery. Prioritise lipids and hematocrit for cardiovascular risk, ALT/AST/GGT on orals, and LH/FSH/testosterone for recovery. Use the sensitive assays for estradiol and free testosterone.
