Researchers reporting in the Journal of Medical Internet Research (2024) bought semaglutide from illegal online sellers that ship without a prescription, then subjected the delivered product to laboratory analysis. Of six vendors selected for test purchases, three delivered injection vials; the Ozempic pens ordered were never delivered (non-delivery scams).
Measured peptide purity in the three delivered samples was 7.7%, 8.97%, and 14.37%, against the 99% purity claimed on labeling. Endotoxin, a bacterial pyrogen that can trigger fever, inflammation, or sepsis when injected, was detected in all three samples at 2.16 to 8.95 EU/mg. The lyophilized samples contained no viable microorganisms at the time of testing, indicating the endotoxin reflects contamination during manufacturing rather than live infection.
On visual and documentation checks, the products failed 59-64% of 22 evaluated quality criteria and were classed as probable substandard and falsified products. The authors noted the measured active-ingredient content in some samples exceeded the labeled amount, compounding the risk of unpredictable dosing on top of the contamination.
The finding is relevant well beyond semaglutide: the same gray-market channels supply research peptides, GLP-1 analogues, and other injectables marketed as not for human use, where no purity, sterility, or endotoxin testing is guaranteed. Endotoxin in particular is not removed by ordinary reconstitution and is not detected by the purity claims vendors advertise.
