Veru announced on July 27, 2026 that it had completed enrollment of its Phase 2b PLATEAU trial, which pairs the selective androgen receptor modulator (SARM) enobosarm with the GLP-1 receptor agonist semaglutide. The trial enrolled approximately 200 patients aged 65 and older with obesity (BMI at or above 35), exceeding its target. Participants are randomized to enobosarm 3 mg or placebo, with all patients initiating semaglutide.

The primary endpoint is the percent change in total body weight at 68 weeks. Secondary measures assessed by DXA and function testing include total fat mass, lean mass, stair-climb power, mobility, bone mineral density, and HbA1c. An interim analysis at 32 weeks is expected in Q1 2027, with topline results anticipated in Q4 2027.

PLATEAU follows the earlier Phase 2b QUALITY study, which enrolled 168 patients aged 60 and older receiving semaglutide. In QUALITY, adding enobosarm produced a statistically significant reduction in lean mass loss versus semaglutide alone (p=0.002) over the 16-week active period, alongside greater fat loss. Veru has also reported that in an extension phase, enobosarm monotherapy limited the regain of weight lost after the GLP-1 drug was stopped.

The rationale reflects a recognized trade-off with incretin therapy: rapid weight loss on GLP-1 and dual/triple agonists includes a substantial fraction of lean (muscle) mass, a concern that is amplified in older adults. Enobosarm — the compound also known as ostarine or MK-2866, long sold on the gray market as an unapproved 'muscle-building' SARM — is being repositioned here as an adjunct intended to shift the composition of weight loss toward fat.

Enobosarm is not approved for any indication. PLATEAU is a Phase 2b study, not a registrational trial, and its total-body-weight primary endpoint over 68 weeks will not report until late 2027; the muscle-preservation and physical-function outcomes are secondary. Whether the lean-mass effects seen at 16 weeks in QUALITY translate into durable functional benefit over a longer, larger trial remains to be demonstrated.
