BPC-157 and TB-500 are the two peptides driving the 'healing peptide' wave across bodybuilding forums, YouTube and TikTok, promoted for rapid recovery of tendons, muscle, gut lining and joints. BPC-157 is a synthetic fragment associated with angiogenesis and local tissue repair; TB-500 is a synthetic fragment related to the protein thymosin beta-4, marketed for systemic recovery and reduced inflammation.
The evidence gap is the central fact. The efficacy case for BPC-157 rests almost entirely on rodent studies; there are no large-scale, randomized, placebo-controlled human trials, and an early human safety trial was reportedly halted without published results. TB-500 has effectively zero completed human clinical trials, and most underlying research concerns the full thymosin beta-4 protein rather than the fragment being sold. A 2024 analysis has even questioned whether TB-500 as sold is the biologically active moiety at all.
Regulators have kept both in a grey zone. The FDA placed BPC-157 and TB-500 in its restricted Category 2 for compounding in 2023 over immunogenicity, impurity and characterization concerns, and its Pharmacy Compounding Advisory Committee has continued to review them for the 503A bulks list — meaning neither is an approved drug or a lawful dietary-supplement ingredient in the U.S.
Honest risk assessment: reported short-term human side effects are usually mild (injection-site reactions, lightheadedness), but there is no long-term human safety data. Two specific cautions recur among clinicians: BPC-157's angiogenic activity is theoretically undesirable for anyone with undiagnosed cancer, and independent testing of grey-market vials has repeatedly found mislabeling, wrong sequences and heavy-metal contamination. Confidence in the healing claims is low and rests on preclinical models.
